I am looking for a way to make a customer survey form pop up for new visitors via either Thickbox or Shadowbox JS.

I'd rather have this happen with a page rather than a post, but regardless, I'm testing with a post for now, but all I get is a blank (all black) Thickbox or Shadowbox JS popping up.
